2018-2019 Student Device Loan Forms


The following loan forms are only for the current 2018-2019 school year. Updated loan forms for the 2019-2020 school year will be posted at the end of this school year.

Students participating in the Digital Learning Movement will be issued a mobile device, charging cables, and a case upon the completion of the program requirements described below. The device will be assigned to the individual student and the serial number recorded. This device will stay with the student for the remainder of the school year in which it was issued or until the student withdraws from school. Every attempt to give students the same device from year to year while attending the same school will be made (with the exception of elementary schools).

Grades 7-12

Step 1: Students must complete 2018-2019 Grades 7-12 Student Device Loan Form online. 

Step 2: Pay the Technology Usage Fee online. If new students do not show a fee online, they may need to pay in person at the school.

Step 3:
 A device will be issued to the student through the school's technology office located in or near the school's media center.

Grades 4-6

Step 1: Students must complete 2018-2019 Grades 4-6 Student Device Loan Form online. 

Step 2: Pay the Technology Usage Fee online. If new students do not show a fee online, they may need to pay in person at the school.

Step 3: A device will be issued to the student through the school's technology office located in or near the school's media center.


Grades K-3

Students in grades K-3 will use devices in the classrooms only. No forms are required.

Student Day User / Opt-Out Form

Flagler County Public Schools has created our Digital Learning Movement to allow all students 24/7 access to the latest technology, that will help prepare them for a technology-driven world. To have the greatest influence on our students’ education, Flagler County Public Schools recommends that parents allow students to take their assigned device home.

The District also understands that some parents may not want their children to bring their assigned devices home. In this event, the student will be a “Day User” and will use the device while on campus. Parents/guardians will complete the 2018-2019 Student Day User form. Day Users may not bring the assigned device home. The parent/guardian/student will be responsible for all damage or loss of the device. If a student takes the device off campus, as a day user, disciplinary action will be taken against the student and the device will be reported stolen.
